Declaration for entities seeking accreditation

•   I declare that I have been authorised by my organisation to make the declarations
    and attestations contained in this application.

•   I declare that all the information that I have provided in this application, including
    any documents, is complete and correct.

•   I understand that, if accredited, my organisation will be required to comply with all
    the requirements in the Digital ID Act 2024, Digital ID Rules, Accreditation Rules
    and Accreditation Data Standards. This includes an obligation to notify the Digital
    ID Regulator of ‘reportable incidents’, and to submit annual reports for each
    reporting period.

•   I declare that the applicant is not aware of any circumstances that might prevent
    or adversely affect my organisation’s ability to comply with the Digital ID Act 2024,
    the Digital ID Rules, Accreditation Rules or the Accreditation Data Standards.
