Declaration for compliance for all entities applying to
participate in the AGDIS
I,                                                of

declare that:

     • I am authorised by my organisation to make the declarations contained in this
     application;

     • I understand that my organisation:

           o is required to comply or will be required to comply if this application is
             approved with all relevant Digital ID legislation (including the Digital ID
             Act 2024, the Digital ID Rules 2024, the Accreditation Rules 2024, and
             the Digital ID Data Standards); and

           o will enter a statutory contract with each other approved accredited
             entity and participating relying party in the AGDIS should it be
             approved.
