AGDIS exemptions for Participating Relying
Parties form
Information provided will be shared with the System Administrator to assist it to
fulfill its powers and functions under the Digital ID Act.
Your organisation is required to submit a signed ‘final declaration’ with this
application.


1. Explain why your organisation is seeking an exemption to voluntariness
   requirements.


2. Can users access the service by other means (e.g by not using a digital ID)?

      Yes            No

   If yes, please explain.


3. Provide supporting evidence which may include justification and other relevant
   documents applicable to the exemption request.
