Revoke Accreditation and/or Approval
Application Form
 •   This form is used by Accredited entities and/or entities who are approved to
     participate in the AGDIS to revoke their accreditation/approval to participate.
 •   Through this form entities can seek to revoke their accreditation or approval
     for all services type(s) or one or more service types that they are accredited/
     approved to provide e.g. an accredited service type such as an accredited
     attribute service provider.
 •   When applying to have accreditation/approval revoked, an entity may provide
     a timeline for when the revocation should apply but the date of revocation will
     ultimately be the decision of the Digital ID Regulator.
 •   If the entity applies to revoke their accreditation, the Regulator must revoke
     their approval to participate in the AGDIS.
 •   Please be aware that the system is only classified to hold information at
     official sensitive level and this should be considered when entering
     information into the application.
 •   Refer to the ACCC’s Privacy Policy which sets out what we will do with your
     information when you submit an application.
