Suspend Accreditation and/or Approval
Application form
 •   This form is used by Accredited entities and/or entities who are approved to
     participate in the AGDIS to suspend their accreditation and/or approval to
     participate.
 •   Entities can seek to suspend a specific service type (service role), or all service
     types that they are accredited/approved to provide.
 •   Suspension can be for a time limited period, or it can be open ended.
 •   Applying to have a service suspended does not automatically guarantee that it
     will be granted by the Digital ID Regulator. When considering the application, the
     Digital ID Regulator will have regard to the impact that suspending the service
     will have on the Digital ID ecosystem.
 •   Please be aware that the system is only classified to hold information at official
     sensitive level and this should be considered when entering information into the
     application.
 •   Refer to the ACCC’s Privacy Policy which sets out what we will do with your
     information when you submit an application.
