Technical Testing Attestation statement for
Accreditation

I,                                           of

hereby attest that:
(a) I am my organisation’s accountable executive and have authority to attest to the
    matters contained in this attestation,

(b) the technical testing required by the Digital ID Accreditation Rules has been
    conducted, and

(c) I am satisfied the results of the technical testing conducted by my organisation
    demonstrate that the information technology system through which my
    organisation will provide its accredited services, if accredited, has the
    functionality to meet the requirements set out in the Digital ID Accreditation
    Rules, and

(d) where applicable (strike through if not applicable), I am satisfied that the
    penetration testing covers the kinds of penetration testing required by the Digital
    ID Accreditation Rules.

I acknowledge that if I, or any other authorised contacts, provide or have provided
false or misleading information, including by omission, that this may result in this
application being denied or suspended until further notice, and any accreditation
granted from this application being revoked, or in enforcement action being taken.
