Application to vary Accreditation or AGDIS
Approval form
   •   This form is used by Accredited entities/entities who are approved to
       participate in the AGDIS to change the name of the entity as it is displayed on
       the Accredited Entities Register/AGDIS register.
   •   Entities can seek to change the name of the entity, as well as the specific
       services that they are accredited/approved to provide.
   •   Please be aware that the system is only classified to hold information at
       official sensitive level and should be considered when entering information
       into the application.
   •   Refer to the ACCC’s Privacy Policy which sets out what we will do with your
       information when you submit an application.
Detail existing names that you are applying to change


Provide new name which is to be displayed in the Accredited Entities Register and if
applicable the AGDIS Register

 If yes, Provide supporting reasons for effective date request


Provide attestation from accountable executive supporting this application
