Application to vary participation start date
for AGDIS approval
Information provided will be shared with the System Administrator to assist it to
fulfill its powers and functions under the Digital ID Act 2024.

Your organisation is required to submit a signed ‘final declaration’ with this
application.

1. Please detail the relevant AGDIS Approval Notice your organisation wishes to
   vary.


2. Explain why your organisation cannot participate on the participation start date.


3. What is the earliest date your organisation can participate from?


4. Provide any other supporting evidence your organisation would like the Digital ID
   Regulator to consider.
